Miss Child's Disclosure Document

Miss Child's Sixth Grade Class - Room 111

Helpful things to know about Sixth Grade...The following information should answer most of your questions and help you know what to expect throughout the year.

Attendance/Make-Up Work: Attendance at school is very important. Help your child be on time and experience the full day. If your child is absent from school, assignments given during that time will be marked “absent” and dated. This work is due within 3 days after your child returns from being absent. If your child is gone for more than a day, you or a sibling are welcome to pick up any work. If your child is going to be missing school, be sure to let the office know by 9:00 a.m. (See the school’s policy on tardiness and absences.)

Supplies and/or helpful donations: All necessary supplies will be provided. Please do not send extra folders or loose-leaf binders. Space in the student desk is very limited. A backpack is important for carrying the Student Planner, homework & library books back and forth. Students will need a set of EARBUDS to use throughout the year!

DONATIONS ARE MOST WELCOME and will assist with replacing depleted supplies. Here are the things sixth graders use and need replacing throughout the year: Earbuds, pencils (pre-sharpened) & eraser tops, colored pencils (pre-sharpened), glue sticks, highlighters, fine pt. black felt tip markers, and black dry erase markers. General classroom needs.... Kleenex!! Clorox wipes!! Hand Sanitizer (the kind you don’t wash off)!!, and paper towels. THANKS!

Water Bottles are welcomed! Water is great for the brain -- and it is hot during the first many weeks of school. Bottles may be kept underneath individual desks. POP-TOP water bottles are the best to help avoid spills! Please replace or keep them clean as needed. They will be removed if they become a distraction.

Book Orders: Scholastic book orders will be available periodically through the year giving you and your child an opportunity to choose supplemental reading materials. There is no obligation to participate, but it does increase a child’s motivation and enthusiasm for home reading. Watch for on-line ordering information to come home each time a book order is opened for ordering.

Birthdays: Your child will be recognized on his/her birthday and receive a surprise from the teacher! You are welcome to send or bring treats, but it is not necessary. Children with summer birthdays will be recognized sometime during the first weeks of the school year.

Student Planner/Homework Organizer (Binder)… The “Student Planner & Homework Organizer” (Binder) will come home every day. The Goal: to help students learn to be responsible, organize, pace, and complete quality assignments on time. For the “BINDER”, at the beginning of each week your child will receive a new “weekly” planner page with assignments and due dates given for the week.

Additionally, students will receive a hard copy of “The Weekly Bulletin” with announcements, updates and class calendar. Parents will receive a digital copy of the Bulletin and Student

Planner page attached to a weekly email keeping you informed about what is happening in sixth grade.

In the binder you can expect to find regular homework assignments with supporting materials, weekly spelling lists & reading logs, country report assignments, book report information, current event report information, and work not completed in class that should be finished and returned the next day. (NOTE: In most cases, plenty of time is given to finish daily work at school. If you see incomplete work coming home often, it means we need to discuss your child’s work habits. Homework is an opportunity to practice and reinforce what is learned in class, as well as practice responsibility.)

SEE Copies of “Homework Expectations” & “Late Work Policy”

Independent Home Reading Log: Independent home reading should be recorded on the Reading Log given to students at the beginning of each week, totaled, signed, and submitted according to the due date (every Monday or Tuesday). NOTE: 6th graders are required to read a minimum of 30 min. EACH school night.

Home/School Connections: We are looking forward to working with you and your child to make this year a positive learning experience. It is important that “we” work together as a team to give your child the best opportunity for success. “Open communication” is crucial. If there is something that we need to be aware of concerning your child, please feel confident to speak openly so that needs and concerns may be effectively addressed. There are multiple ways to communicate your concerns, questions and needs.

• Send a note or respond to the weekly email.

• Call the school and leave a message -- if it is an emergency, please indicate.

• SEP conferences are an opportunity to come and discuss your child’s progress. You will receive information about when and how to sign up for a conference.

• Email. Emails are checked often throughout the school day and will be responded to ASAP for all communications.

NOTE: Cell Phones/iPods/Smart watches/Other Personal Electronic Devices -- Please review the school copy of “Possession and Use of Personal Electronic Devices by Students found on the school webpage.

Curriculum notes: DESK (Davis Essential Skills & Knowledge) -- The 6th grade curriculum is based on the DESK. The purpose of the DESK is to focus and prioritize the Utah State Standards that students are expected to learn. (See a copy of the Sixth Grade DESK on the district web page.)

Specialty Classes: P.E., Music, Computer Technology, and Library skills are all taught by specialists. Your child will participate in each subject once a week on an assigned day. Students need to be prepared for each subject. (i.e. proper shoes for P.E., library book, etc.)

Assessment/Report Cards -- Students’ grades will be calculated & reported using the Davis District’s “SMART” System (Standards Mastery Assessment Reporting Tool). All assignments/projects will be graded with points or percent which are automatically converted to a rubric score of 1, 2, or 3 for the report card (3 indicating on grade level). Completing assignments on time with quality effort is the BEST! Late work is better than missing work. Missing work becomes a zero with no credit. Please assist your child with being responsible for completing work with quality effort and turning assignments in on time! Recognizing that "life happens" sometimes, four late coupons are provided for your child to use EACH term (see copies of “Late Work” policy & “Homework Expectations”).

Your myDSD Account – Students & parents can access and review completed and/or missing assignments & scores on a regular basis throughout each term by logging into your individual myDSD account. At the end of each term, the report card will be available for reviewing online through your myDSD account. For instructions on creating an account, visit the Davis School District webpage, click the myDSD link in the upper right-hand corner under “Tools” and follow the instructions. Parents and Students will find the myDSD account to be a very valuable tool in monitoring progress, checking assignment grades, and checking for missing work. This tool should be utilized on a regular, weekly basis.

Parent Volunteers: If you are volunteering in our classroom, be sure to check-in at the office as a volunteer and obtain a visitor/volunteer badge before coming to our classroom. There are many ways to help out this year. PLEASE watch for opportunities to volunteer in the weekly email. To volunteer, just reply to the email indicating your willingness. You will receive a confirmation email with additional information. Thank you in advance!!

NOTE: All volunteers MUST complete a background check through Davis School District office PRIOR to any volunteering in the school! Thanks!!

WHEW!! That was a lot of reading, but you made it!! Looking forward to a GREAT year!!
